Alternatively: "Yes, okay. You?" "Better, thanks." "Not so great." "Good." "Okay really." 'Okay' is a useful one; it allows for ambiguity. Even when I use it to indicate its opposite, the conversation can go on. Saying 'I'm better' is saying what the other wants to hear; it is, most of the time, a lie. 'Good' I want to save for when that's true. 'Not so great' is for those I sense might understand. But - how are you?
