{"id":1366,"date":"2024-09-02T16:06:14","date_gmt":"2024-09-02T15:06:14","guid":{"rendered":"https:\/\/wp.lancs.ac.uk\/digi-age\/?p=1366"},"modified":"2024-09-02T17:29:23","modified_gmt":"2024-09-02T16:29:23","slug":"digitising-health-and-social-care-why-a-technofix-wont-do","status":"publish","type":"post","link":"https:\/\/wp.lancs.ac.uk\/digi-age\/2024\/09\/02\/digitising-health-and-social-care-why-a-technofix-wont-do\/","title":{"rendered":"Digitising Health and Social Care \u2013 Why a Technofix Won\u2019t do"},"content":{"rendered":"<p><span data-contrast=\"none\">The COVID-19 pandemic has accelerated the development and speed of adoption of digital technology in <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9499383\/figure\/fig1\/\"><span data-contrast=\"none\">every sphere<\/span><\/a><span data-contrast=\"none\"> of healthcare, including individual health management and care, population-wide health approaches, and the real-time generation, management and integration of medical data. Discussions of healthcare futures are suffused by references to \u2018digital transformations\u2019 and \u2018digital transitions\u2019, where digital technology is often looked upon as the panacea to the global \u2018crisis of care\u2019 and the challenges posed by \u2018ageing populations. This can run the danger of the widening gap between care needs and available resources being <\/span><a href=\"https:\/\/www.bmj.com\/content\/380\/bmj.p464\"><span data-contrast=\"none\">oversimpl<\/span><\/a><span data-contrast=\"none\">ified<\/span><span data-contrast=\"none\"> as a problem of <\/span><span data-contrast=\"auto\">organisational deficiency, inefficiency, poor information management, outdated technology, and a shortage of staff.<\/span><span data-contrast=\"none\"> Although these are very real problems and digital technology holds great potential here in improving matters, their underlying causes run the danger of remaining unaddressed by focusing solely on technology as the silver bullet.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">The <\/span><a href=\"https:\/\/www.england.nhs.uk\/digitaltechnology\/\"><span data-contrast=\"none\">digital transformation of the National Health Service (NHS)<\/span><\/a><span data-contrast=\"none\"> in England, encompassing the digital transformation of health- <\/span><i><span data-contrast=\"none\">and<\/span><\/i><span data-contrast=\"none\"> social care<\/span><span data-contrast=\"auto\"> (the latter providing people with practical assistance and personal care if they need it)<\/span><span data-contrast=\"none\">, is a case in point. <\/span><span data-contrast=\"auto\">In 2023, the number of people on waiting lists for hospital treatment in England peaked at 7.8 million, while over 2.6 million people over the age of fifty were on waiting lists for social care, <\/span><a href=\"https:\/\/www.ageuk.org.uk\/our-impact\/campaigning\/care-in-crisis\/#:~:text=2.6%20million%20people%20in%20England,t%20do%20these%20things%20unaided.\"><span data-contrast=\"none\">\u2018left struggling to go to the toilet, eat, get dressed or wash<\/span><\/a><span data-contrast=\"none\">\u2019<\/span><span data-contrast=\"auto\">. Research by Age UK showed that between 2022 and 2023, 28,655 adults over the age of sixty-five died before they reached the top of this list and received the care they needed. This equates to an average of 79 deaths a day, 550 deaths a week, and 2,388 deaths a month.\u202fTo tackle such backlogs, partly caused by the chronic staff shortages and high staff turnover besetting the sector, the digital transformation of health and social care is looked upon here <\/span><span data-contrast=\"none\">as <\/span><span data-contrast=\"auto\">the <\/span><a href=\"https:\/\/publications.parliament.uk\/pa\/cm5803\/cmselect\/cmhealth\/223\/report.html\"><span data-contrast=\"none\">solution<\/span><\/a><span data-contrast=\"auto\"> to <\/span><a href=\"https:\/\/commonslibrary.parliament.uk\/capacity-pressures-in-health-and-social-care-in-england\/#:~:text=2024%20general%20election.-,Record%20demand%2C%20an%20ageing%20population%20and%20inflation%20are%20all%20squeezing,general%20and%20acute%20hospital%20beds.\"><span data-contrast=\"none\">systemic problems<\/span><\/a><span data-contrast=\"auto\"> besetting the healthcare system. Regarding staff shortages, for example, finding a techno-fix must seem far easier to policymakers than addressing the root causes of what professor of philosophy and politics Nancy Fraser has referred to as the \u2018<\/span><a href=\"https:\/\/www.dissentmagazine.org\/article\/nancy-fraser-interview-capitalism-crisis-of-care\/#:~:text=This%20%E2%80%9Ccrisis%20of%20care%E2%80%9D%20should,present%20regime%20of%20financialized%20capitalism.\"><span data-contrast=\"none\">social-reproductive crisis<\/span><\/a><span data-contrast=\"auto\">\u2019 caused by capitalist relations of production. Hence policies like England\u2019s 2021 white paper <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/people-at-the-heart-of-care-adult-social-care-reform-white-paper\"><span data-contrast=\"none\">People at the Heart of Care<\/span><\/a><span data-contrast=\"auto\"> and the <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/adult-social-care-system-reform-next-steps-to-put-people-at-the-heart-of-care\/next-steps-to-put-people-at-the-heart-of-care\"><span data-contrast=\"none\">2023 Next Steps paper<\/span><\/a> <span data-contrast=\"auto\">look to the \u2018transformative power\u2019 of digital technology to improve the quality, safety and personalisation of care. However, this transformative power has many different sides, not all of which are necessarily positive.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">While not too long ago, <\/span><span data-contrast=\"auto\">a patient\u2019s first point of contact with the National Health Service (NHS) would have been a receptionist, spoken to either in person or over the phone, today, this is likely to be the NHS app, <\/span><span data-contrast=\"none\">widely regarded as the \u2018front door\u2019 of the NHS. It has over <\/span><a href=\"https:\/\/www.england.nhs.uk\/2023\/05\/number-of-repeat-prescriptions-ordered-via-nhs-app-up-by-92-in-the-last-year\/#:~:text=The%20NHS%20App%20has%20now,securely%20through%20the%20NHS%20App.\"><span data-contrast=\"none\">28 million people subscribe<\/span><\/a><span data-contrast=\"none\">rs<\/span> <span data-contrast=\"none\">who use it to access health- and social care services, check <\/span><span data-contrast=\"auto\">medical records, book appointments, or order prescriptions. Between October 2021 and September 2022, 1.2 billion people also visited the <\/span><a href=\"https:\/\/digital.nhs.uk\/news\/2022\/1.2-billion-visits-to-nhs-website-in-last-12-months\"><span data-contrast=\"none\">NHS website<\/span><\/a><span data-contrast=\"auto\">, primarily to find Covid-related information. Today, an average of twenty-three <\/span><span data-contrast=\"none\">million people access what has become Europe\u2019s largest online health information source weekly. While there are many benefits associated with these developments, digital technology, unfortunately, does not <\/span><span data-contrast=\"auto\">benefit everyone equally. Many people don\u2019t know how to use it, lack the financial resources required to purchase it, have no broadband connection, or simply do not wish to make it part of their life.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Older adults, long known to be lesser beneficiaries of the digital economy, are particularly prone to what has come to be referred to as \u2018digital exclusion\u2019. <\/span><a href=\"https:\/\/www.ageuk.org.uk\/our-impact\/programmes\/digital-skills\/\"><span data-contrast=\"none\">25% of people aged over sixty-five in the UK<\/span><\/a><span data-contrast=\"auto\"> do not use the Internet and lack essential digital skills<\/span><span data-contrast=\"none\">, risking access to digital health services, and, increasingly, to services like banking and shopping.\u00a0 Data from the English Longitudinal Study of Ageing (<\/span><a href=\"https:\/\/www.elsa-project.ac.uk\/\"><span data-contrast=\"none\">ELSA<\/span><\/a><span data-contrast=\"none\">) shows that accessing medical appointments, support services, and consumer activities<\/span><span data-contrast=\"auto\"> in the UK was made worse for the digitally excluded during Covid, \u2018further exacerbating the negative health impact of the pandemic, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35885706\/\"><span data-contrast=\"none\">increasing isolation and loneliness, and social exclusion<\/span><\/a><span data-contrast=\"auto\">.\u2019 The <\/span><span data-contrast=\"none\">cost-of-living crisis in the UK has since forced many more people to give up their broadband access to afford essentials. This <\/span><span data-contrast=\"auto\">digital skills gap between age groups is not unique to Britain. Eurostat data collected across all EU member states in 2023 found that only 33% of those aged between fifty-five and seventy-four years of age had basic digital skills, compared to 80% of individuals aged between sixteen and twenty-four. However, older adults are not the only ones lacking digital skills.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">The <\/span><span data-contrast=\"auto\">Lloyds Consumer Digital Index<\/span><span data-contrast=\"none\"> of 2023 found that over <\/span><span data-contrast=\"auto\">54% of working adults in the UK \u2013 including many carers \u2013 don\u2019t know how to perform the 20 tasks that industry- and government representatives have deemed essential for today&#8217;s digital workplace.\u202fLarge numbers of people are, therefore, in real danger of <\/span><a href=\"https:\/\/www.lloydsbank.com\/banking-with-us\/whats-happening\/consumer-digital-index.html\"><span data-contrast=\"none\">being left behind<\/span><\/a><span data-contrast=\"auto\"> in the workplace if digitalization continues apace. This poses serious challenges, including for the health sector, where experts have warned that there is still a long way to go in order to close the digital skills gaps and bring about a successful digital transformation.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Digitalisation in the fields of health and social care, however, is about much more than simply changing how people access the NHS. For example, v<\/span><span data-contrast=\"auto\">irtual care models like \u2018virtual wards\u2019 have the potential to further <\/span><a href=\"https:\/\/blog.hettshow.co.uk\/benefits-and-challenges-of-virtual-wards\"><span data-contrast=\"none\">marginalise vulnerable populations<\/span><\/a><span data-contrast=\"auto\"> and exacerbate existing healthcare disparities. The Health Foundation also believes that inclusive, participatory decision-making will be key to their success as not everyone is convinced that they are the right way forward. Concerns have been growing about the extreme, large-scale risks that transformative technologies like powerful AI systems could pose for society, and in 2024, the Washington D.C.-based <\/span><a href=\"https:\/\/www.caidp.org\/\"><span data-contrast=\"none\">Center for AI and Digital Policy<\/span><\/a><span data-contrast=\"auto\"> warned that the <\/span><a href=\"https:\/\/thehill.com\/opinion\/technology\/4722343-its-time-for-the-ftc-to-act-on-chatgpt\/\"><span data-contrast=\"none\">\u2018<\/span><span data-contrast=\"none\">race to replace people in the workplace, in schools, for elder care and nursing is on\u2019<\/span><\/a><span data-contrast=\"none\">.<\/span><span data-contrast=\"none\"> Amidst the push by governments and private sector industry for the speedy digitalisation of nearly every sphere of human life, calls such calls for more time to explore the long-term consequences of digitalisation struggle to be heard. Innovation today, including digital innovation, is predominantly driven by a privately funded, profit-motivated, <\/span><span data-contrast=\"auto\">knowledge economy<\/span><span data-contrast=\"none\">. This can leave little room for the upstream, public engagement in Research &amp; Development (R&amp;D) processes as those recommended by <\/span><a href=\"https:\/\/www.ukri.org\/who-we-are\/epsrc\/our-policies-and-standards\/framework-for-responsible-innovation\/\"><span data-contrast=\"none\">Responsible Research and Innovation<\/span><\/a><span data-contrast=\"none\"> (RRI) guidelines. Discussions around the development and governance of AI, in particular, are dominated by the private sector industry. This situation has led to pleas for <\/span><a href=\"https:\/\/www.societyandspace.org\/articles\/another-science-is-possible-by-isabelle-stengers\"><span data-contrast=\"none\">a slow science<\/span><\/a><span data-contrast=\"none\"> where scientists and innovators make time for listening to divergent and opposing views about their work, taking them on board, and for the bottom-up co-production of techno-futures where those \u2018<\/span><a href=\"https:\/\/www.adalovelaceinstitute.org\/blog\/meaningful-public-participation-and-ai\/\"><span data-contrast=\"none\">standing the most risk of being harmed\u2019<\/span><\/a><span data-contrast=\"none\"> can have a full say. This particularly includes older adults, and they could once again be the ones drawing the short straw.\u00a0 In <\/span><a href=\"https:\/\/www.epsu.org\/search?search_api_fulltext=Carebots\"><span data-contrast=\"none\">recommendations<\/span><\/a><span data-contrast=\"none\"> recently made to robot device developers reminding them of the importance of stakeholder engagement in decision-making processes around the role of robots in care homes (Carebots), older adults were conspicuous by their absence. Yet, <\/span><i><span data-contrast=\"none\">they <\/span><\/i><span data-contrast=\"none\">will be the ones most affected by them where care homes go down that route.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:360}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Interviews carried out with older adults as part of the DigiAge project underline the need for more people-centred approaches in the design of digital technologies and the decision-making around their governance, regulation and use. Of the sixteen older adults aged between sixty-eight and ninety-two years of age we interviewed in 2024, the large majority were concerned about the pace of technological change, feeling unable to keep up with it. Many were also worried about how their private data was being used, and not in support of the automation of telephone services. Concerning in-home monitoring and ageing at home, some felt trapped. Do people still have a choice? The view of older adults as \u2018dependent\u2019, particularly when monetary costs are associated with looking after them, is a strong driving force behind the push for the digitalisation of health and social care. This negates their own choices for their future whilst, at the same time, turning them into technology consumers, which benefits the economy. However, unchecked digitalisation here, and in other spheres of life, should not obscure the real needs and wants of people.
